Long touted for its cholesterol and vasodilation benefits, niacin seems a worthy supplement. And for those trying to fortify their heart health in their 60s, 70s, and beyond, science highlights the importance of precision when harnessing this double-edged nutrient.
But emerging research suggests there may be some unseen risks rather than rewards…
First, what exactly is niacin?
Also known as vitamin B3, niacin is one of several essential B complex vitamins needed in small amounts for maintaining optimal health.
And, as an indispensable nutrient, niacin plays key roles in converting nutrients from food into cellular energy and aiding DNA production. Alongside vitamins B1, B2, and B12, niacin keeps energy metabolism, nerve, and immune function humming smoothly when consumed regularly through a balanced diet. Food sources rich in niacin include meat, fish, nuts, legumes, grains and some vegetables.
But now, after analyzing blood samples from thousands of cardiac patients, researchers recently discovered links between byproducts of excess niacin breakdown and higher rates of heart attacks, stroke, and related complications over three years.

Niacin’s Dark Side
Available naturally in foods like meat, fish, and wheat bran—or synthetically as a supplement—niacin helps process fats, carbs, and alcohol. It also helps dilate blood vessels when taken appropriately.
Yet high doses of niacin supplements have notoriously caused unpleasant flushing. And excess appears capable of more lasting downsides.
In heart patients followed for years, researchers correlated surging blood levels of two niacin metabolites (called 2PY and 4PY) with significantly heightened incidence of repeat heart attacks, stroke, bypass surgery, and related adverse events.
Men taking niacin showed elevated production of these byproducts, warranting caution for long-term supplements without medical oversight.

Our bodies efficiently process appropriate niacin amounts through Phase I and II detoxification. But repeated overdosing overworks this system, generating excess metabolites not easily cleared.
Studies are still uncovering how niacin metabolite buildup sparks recurring cardiac crises. But early evidence points to inflammation playing a role.
In cells and mice, one metabolite concentration in particular ramped up inflammatory messaging and cellular adhesion activity—early stepping stones towards plaque growth and clots blocking fragile heart vessels.

Instead of taking high doses through supplements, recent data suggests taking a calibrated approach that balances getting enough niacin for heart benefits while avoiding buildup of potentially harmful byproducts.
