A recent JAMA Ophthalmology study reviewed nine cases of people on semaglutide and tirzepatide who developed rare but serious eye conditions, including:

- Paracentral Acute Middle Maculopathy: Damage to the retina, the part of your eye that processes light
Seven of these patients developed optic nerve strokes, one had a retinal stroke, and one had optic nerve swelling—all potentially blinding conditions.
The researchers believe rapid drops in blood sugar may be behind these issues. When blood sugar crashes too quickly, fluid shifts in the eye could trigger inflammation, swelling, or even small strokes in the eye’s delicate tissues.

What This Means for You
To be clear, these cases are rare. Millions of people take GLP-1 drugs without any major eye issues.
But when a drug is being prescribed at record-breaking rates—with 2-3 percent of the U.S. population using them last year alone—we need to pay attention to even the rarest risks.
The researchers behind this study aren’t calling for people to stop taking these medications.
Instead, they’re urging:
- Slow, gradual dose increases to avoid sudden drops in blood sugar
- Ophthalmologist checkups for anyone experiencing vision changes
- More research to fully understand these risks
And that’s the right approach.
If you’re taking (or considering taking) Ozempic, Wegovy, or Mounjaro, you don’t need to panic—but you should be aware of the risks and discuss them with your doctor.

The reality is, no drug is completely safe.
